On 20th August,2019 The Government of Punjab launched Mukhmantri Sehat Bima Yojana (Ayushman Bharat PM-JAY Mukhmantri Sehat Bima Yojana (AB PM-JAY MMSBY) This State flagship Health Insurance Scheme aims to back the population of approximately 65% in the State of Punjab with an OTP based cashless and paperless health insurance cover for secondary-tertiary care hospitalization. The scheme means that legally covered diseases /medical conditions are treated in Government/ Empanelled Private hospitals
Benefits of the Scheme:
- Cashless Health Insurance Cover: Beneficiaries receive coverage of up to ₹5,00,000 per family per year for secondary and tertiary care hospitalization.
- The coverage includes in-patient services for the treatment of listed diseases and medical conditions.
- Cashless and paperless treatment is available at empaneled hospitals.
Eligibility:
The scheme is available to the residents of Punjab who belong to the following categories:
- Families from SECC 2011 data.
- Smart Ration Card holder families.
- J-Form holder farmers.
- Small traders registered with the Excise & Taxation Department.
- Small & Marginal Farmers.
- Accredited & Yellow Card Journalists.
- Construction workers registered with the Punjab Building & Other Construction Workers Welfare Board.
Application Process:
Online Registration for E-Card Generation:
- You can generate the e-card from the comfort of your home by either downloading the ‘Ayushman App’ on your mobile or visiting the official beneficiary portal for self-registration.
- Alternatively, visit any empaneled private or public hospital to get the e-card generated.
- The list of empaneled hospitals is available on the SHA (State Health Agency) website.
Documents Required:
- Aadhaar Card.
- Ration Card.
- Family Declaration (if no ration card is available).
- Income Certificate.
- Construction Worker Card (if applicable).
