Offline Mode
• Step 1: Print the prescribed format of the application form
• Step 2: Fill in all required details, paste latest photographs of the Field of Play with date & location mentioned, and attach all necessary documents
• Step 3: Submit the completed form to the District Sports Officer
• Step 4: Collect an acknowledgement receipt with submission date, time, and unique ID (if applicable)
Note 1: For Municipal Bodies, the application must be submitted by the concerned ward member or councillor
Note 2: For Gram Panchayats, the Sarpanch may apply. If the post is vacant, the Gram Sachiv may submit the application
