Application Process

Procedure for Claiming Benefits (Offline):

  1. Obtaining the Application Form:

    • Form-A: For claims related to natural or unnatural death.
    • Form-B: For claims related to partial or permanent total disability due to an accident.

    • The completely filled claim form must be submitted to the Tahsildar/Deputy Tahsildar of the respective Taluk/Sub-Taluk Office within 180 days from the date of death or the accident.
  4. Verification & Settlement:
    • The Taluk/Sub-Taluk Office scrutinizes the claim form and forwards eligible claims with proper certification to the Settlement Officer.
    • Claims submitted after 180 days may be considered if condoned by the Collector, but incomplete submissions are not accepted.
    • Upon verification, the Settlement Officer sanctions the financial assistance:
      • Form-C: For death-related claims, payments are made via e-payment into the beneficiary’s bank account.
      • Form-D: For disability-related claims, payments are also disbursed via e-payment to the beneficiary’s bank account in any nationalized bank within Puducherry.
